RÉSUMÉ
Contexte. La phase d’étude préprothétique conditionne le diagnostic et le choix thérapeutique en prothèse amovible. Les perturbations occlusales consécutives à l’édentement et à des soins ponctuels successifs sont fréquemment sous-estimées lorsque l’examen clinique est utilisé seul. Objectif. Décrire l’apport diagnostique, thérapeutique et communicationnel du montage directeur réalisé sur articulateur dans différentes situations cliniques, et préciser les principes de sa réalisation. Approche. Revue narrative illustrée par dix figures cliniques (prothèse amovible partielle, prothèse amovible totale, prothèse supra-radiculaire, set-up orthodontique) et appuyée sur des références indexées dans PubMed et/ou Scopus. Résultats. L’analyse des modèles montés sur articulateur a permis d’objectiver des perturbations du plan d’occlusion (égressions, versions), une insuffisance d’espace prothétique vertical et un rapport intermaxillaire de classe III ; de simuler et de quantifier les corrections (améloplasties, set-up, positionnement des dents prothétiques) ; de valider l’esthétique et le soutien labial à l’essayage ; et de trancher entre conservation et extraction de racines résiduelles avant tout geste irréversible. Le montage directeur a également servi de base aux prothèses provisoires et aux guides d’imagerie ou chirurgicaux, et de support de communication avec le patient et le prothésiste. Conclusion. Dans les limites d’un niveau de preuve essentiellement descriptif, le montage directeur constitue un outil rationnel de planification qui transpose à la prothèse amovible la logique « simuler avant de traiter » établie en prothèse fixée. Des études prospectives devraient quantifier son effet sur la précision des traitements, la compréhension du patient et les résultats prothétiques.

ABSTRACT
Background. The pre-prosthetic study phase conditions diagnosis and treatment selection in removable prosthodontics, yet occlusal disturbances arising after tooth loss and repeated piecemeal treatment are often under-recognised when clinical examination is used alone. Objective. To describe the diagnostic, therapeutic and communication value of the guiding tooth set-up (montage directeur) performed on an articulator across several clinical situations, and to clarify the principles of its fabrication. Approach. Narrative review illustrated by ten clinical figures (removable partial dentures, complete dentures, root-supported overdentures, orthodontic set-up) and supported by references indexed in PubMed and/or Scopus. Findings. Analysis of articulator-mounted casts made it possible to objectify disturbances of the occlusal plane (egression, tilting), insufficient vertical prosthetic space and a Class III intermaxillary relationship; to simulate and quantify corrections (selective reshaping, set-up, positioning of prosthetic teeth); to validate aesthetics and lip support at try-in; and to decide between retention and extraction of residual roots before any irreversible step. The guiding set-up also provided the basis for provisional prostheses and for imaging or surgical guides, and served as a communication tool with the patient and the dental technician. Conclusion. Within the limits of a predominantly descriptive evidence base, the guiding set-up is a rational planning tool that transposes to removable prosthodontics the “simulate before you treat” logic established in fixed prosthodontics. Prospective studies should quantify its effect on treatment accuracy, patient understanding and prosthetic outcomes.
